Privacy Policy

Information we collect

To provide safe dental care we collect personal information including your name, contact details, date of birth, Medicare and health fund details, emergency contacts, and payment information.

We also collect health information: your medical and dental history, medications, allergies, clinical notes, radiographs and scans (including CBCT 3D imaging), photographs, referral letters and specialist correspondence. This information is essential — without it we may be unable to treat you safely.

How we collect it

Information is collected directly from you through enquiry and booking forms on this website, over the phone, in person at our practices, and during the course of your treatment. With your consent, we may also collect information from other treating practitioners, specialists, your health fund or Medicare.

When you submit an enquiry form on this site (for example, the 'See if you qualify' form), the details you provide are used to contact you about your enquiry and arrange your consultation.

How we use your information

Your information is used to provide and coordinate your dental care, communicate with you about appointments and treatment, process payments and health fund claims, prepare documentation you request (such as reports supporting an application for early release of superannuation), and meet our legal and professional record-keeping obligations.

With your consent, we may send you appointment reminders, recall notices and practice updates. You can opt out of non-essential communications at any time.

When we disclose information

We only disclose your information where needed for your care or where the law requires it: to specialists and health practitioners involved in your treatment, dental laboratories, radiology providers, your health fund, Medicare or the ATO (for example, in support of a compassionate release of super application you initiate), our practice software and secure IT providers, and where required by law or a professional obligation.

We do not sell your personal information to anyone.

Storage and security

Records are held in secure, access-controlled practice management systems used by Grey Street Dentist and Synergy Dental Group. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification or disclosure — including staff confidentiality obligations, access controls and secure backups.

Website, cookies and analytics

Our website may use cookies and analytics tools to understand how visitors use the site and to improve it. This data is aggregated and does not identify you personally. Embedded services on this site (such as Google Maps) may collect data in accordance with their own privacy policies.

Access and correction

You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate or out of date. Contact the practice you attend and we will respond within a reasonable time. In limited circumstances access may be declined where the law allows; if so, we will explain why.

Complaints and contact

If you have a question or concern about how your information has been handled, please contact us first so we can resolve it: Grey Street Dentist, Suite S2/18 Grey Street, St Kilda VIC 3182, (03) 9534 4017 — or Synergy Dental Group, 100 Lower Dandenong Rd, Parkdale VIC 3195, (03) 7036 6855.

If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or the Health Complaints Commissioner (Victoria).
